Support Coordinator Are you passionate about helping others live fuller, more independent lives? We're looking for a compassionate, organized, and motivated professional to join our team as a Support Coordinator serving individuals with behavioral health needs and developmental disabilities. This is a rewarding opportunity to make a meaningful difference by connecting individuals and families with essential services, resources, and ongoing support within their communities. Qualifications Bachelor's degree min. required Must be able to pass a Level 2 background screening Minimum of 2 years of experience working with individuals with behavioral health needs and/or developmental disabilities Position Details Initial training will be completed at our office located in Haines City Client home visits are required based on individual living settings, with most visits occurring no more than once per month Flexible, community-based work environment Make your schedule as needed consumer locations are based in Polk, Highlands, Hardee counties. Responsibilities Maintain accurate and up-to-date progress notes and central files within the iConnect system Coordinate and monitor services provided by multiple care providers and agencies Assist individuals in accessing community resources, programs, and support networks Advocate for the rights, goals, and needs of the individuals served Build positive relationships with clients, families, and community partners What We're Looking For The ideal candidate is compassionate, dependable, detail-oriented, and committed to improving the quality of life for others.
