Job Description

The Support Coordinator plays a central role in supporting participants by serving as the primary point of contact for services and coordination. This position works collaboratively with participants, families or legal representatives, BHSSC staff, and Conflict Free Case Managers to ensure services are delivered effectively, rights are upheld, and required documentation and follow-up are completed accurately. The role blends relationship-building with organization and accountability, offering the opportunity to make a meaningful, day-to-day impact while supporting person-centered outcomes. Black Hills Special Services Cooperative (BHSSC) is a public entity that is an educational cooperative under the State of South Dakota.   • Manage an assigned caseload and serve as the primary BHSSC contact for individuals receiving developmental disability services, ensuring services remain person-centered and responsive to individual needs.
Coordinate person-centered planning and support services, working closely with individuals, families, legal representatives, Conflict-Free Case Managers, healthcare providers, and other team members. Monitor implementation and effectiveness of Individual Support Plans (ISPs), ensuring goals, outcomes, services, and supports are delivered as intended and updated when needed. Maintain accurate documentation, assessments, and records in Therap and other agency systems while ensuring compliance with HIPAA, Medicaid requirements, and agency policies. Promote and protect individual rights, informed choice, and due process, including completing rights assessments, monitoring restrictions, and supporting rights restoration when applicable. Respond to complaints, concerns, and service issues, ensuring concerns are addressed appropriately, documented, and resolved in accordance with BHSSC policies and procedures. Support positive behavior and quality-of-life outcomes by coordinating Positive Behavior Support Plans and collaborating with interdisciplinary teams to address behavioral, health, and service-related needs. Monitor incidents, significant changes, risks, and safety concerns, ensuring appropriate reporting, follow-up, trend analysis, and communication with relevant team members. Facilitate communication and collaboration across service teams, maintaining strong relationships with participants, families, staff, community partners, and leadership while ensuring coordinated service delivery. Ensure regulatory compliance and support quality assurance efforts, including adherence to state and federal requirements, participation in reviews and audits, and continuous quality improvement activities.
