Helps with rehabilitation for people with disabilities or those in recovery, or coordinates home care services for the elderly; may also coordinate housing or job training assistance for immigrants, veterans, former inmates or families in crisis.

FULL-TIME
Monday•Thursday at 8:00 AM•4:30PM Pay Range:
$27.00•29.00/hour, depending on experience To be successful in the role, you would have: 1+ years of experience in case management required. 2+ years of social services experience, preferably in a senior care setting A Bachelor's Degree required in social work or related program such as gerontology, psychology, or recreation therapy Social Services certification preferred What a day in the life may include: Assessing resident needs based on admission social services history, resident's health, psychological and social needs Promoting a positive living experience for our residents Ensuring confidentiality by adhering to HIPPA & all other privacy standards Promoting a positive work environment by collaborating with community team members What's in it for you?
