!!! Progressive Healthcare Providers (PHP) is an agency providing independent living to adults with intellectual, developmental and physical disabilities. We need to a a Case Manager to our team of caring and professional staff.

Description:

The Case Manager/Qualified Intellectual Disabilities Professional (QIDP) will serve as an integral member of the Program Team and will assist in development, implementation and monitoring of individualized service plans for adults with developmental disabilities. You will coordinate care, train staff, manage compliance with regulations, and ensure active treatment to help individuals achieve personal goals.

Responsibilities:

Ensure individual assessments are completed by Professionals/Consultants to determine the correct level of functioning within thirty (30) days of admission, at least annually and as needed. Facilitate the exchange of information amongst the interdisciplinary team to review and revise individual programs, schedules, strategies, and progress as needed. Ensure completion of all documentation regarding programming, health and safety, comprehensive assessments, including daily observation, log sheets, and daily reports. Train staff on multiple in-services, program implementation and other subjects using demonstrations, practice, observations and feedback methods to ensure staff mastery of technique is achieved. Complete Monthly, Quarterly and other assigned progress reports for all programs.

Requirements:

Bachelor's Degree in Human Services or Behavioral field required. Must meet requirements for title of QIDP according to ADMH standards. Valid Driver's License Strong MS Office Suite Knowledge. Apply Today! PHP is an equal opportunity employer
