The Social Service Director leads the Social Services Department and ensures our residents' medical, emotional, and social needs are supported in accordance with state and federal guidelines.

Key Responsibilities:

Develop, implement, and monitor social service programs that meet all regulatory requirements Support discharge planning, resident assessments, and individualized social care plans Collaborate with residents, families, healthcare providers, and community organizations Coordinate department functions and manage social services staff Ensure compliance, resident rights, confidentiality, and quality standards Participate in quality improvement efforts and handle social services emergencies

Qualifications:
