Job Title:
Registered Nurse or Licensed Practical Nurse -
Social Services Support Job Summary:
We are seeking a compassionate and experienced registered nurse or licensed practical nurse to support our social services team in a long-term care setting. The successful candidate will collaborate with residents, families, and healthcare teams to ensure comprehensive care planning, assessments, and discharge coordination. This role involves providing emotional support, facilitating communication between residents and their families, and assisting with resident assessments and documentation to promote quality of life and smooth transitions.
Responsibilities:
Perform assessments of residents at admission, upon change of condition, and annually; assist in creating, reviewing, and updating care plans and progress notes. Support residents and their families in coping with placement in a skilled nursing facility, including addressing physical illnesses, disabilities, and emotional needs. Coordinate with nursing staff to schedule admissions, transfers, and discharges of residents. Assist in scheduling outside services such as dental or optical appointments for residents. Participate in resident counsel meetings and document discussions accurately. Work collaboratively with the healthcare team to develop discharge plans that meet residents' needs. Ensure all job responsibilities are performed in accordance with applicable laws, professional standards, and organizational policies. Perform additional duties as assigned to support resident well-being.
Qualifications:
Current licensure as a registered nurse or licensed practical nurse required. Experience working in a long-term care or similar healthcare environment preferred. Strong assessment skills and the ability to coordinate multidisciplinary care. Excellent communication skills to effectively interact with residents, families, and healthcare team members. Ability to maintain confidentiality and adhere to professional standards and legal requirements. Compassionate approach to resident care with a focus on quality of life.
