Mission Palms Post Acute is seeking a compassionate, organized, and dedicated Director of Social Services to join our interdisciplinary team. Position Summary The Director of Social Services is responsible for providing emotional support, care planning, discharge coordination, and advocacy for our residents and their families. This role works closely with nursing, therapy, physicians, and community resources to ensure residents receive the highest quality of care throughout their stay. Qualifications Minimum of 1 year of experience in Long-Term Care Social Services or Discharge Planning is required. Knowledge of Medicare, Medicaid, and discharge planning processes. Excellent communication, organization, and problem-solving skills. Ability to build strong relationships with residents, families, and interdisciplinary team members. Knowledge with Point Click Care is a plus! Responsibilities Complete psychosocial assessments and develop individualized care plans. Provide emotional support and counseling to residents and families. Coordinate safe and timely discharges. Assist with advance directives, resident rights, and family meetings. Connect residents and families with community resources. Maintain accurate documentation and ensure regulatory compliance. Participate in care conferences and interdisciplinary meetings. Promote resident dignity, quality of life, and overall well-being.
