Full-Time | 99-Bed Skilled Nursing Facility (SNF)

Pay:

$28.00 per hour - Negotiable Based on Experience We are seeking an experienced and compassionate Certified Social Services Director to join our team at our 99-bed skilled nursing facility. Required Qualifications Current certification as a Social Services Director is required Previous Social Services Director experience in a Skilled Nursing Facility (SNF) is required Knowledge of SNF regulations, resident rights, and compliance requirements Experience with care planning and interdisciplinary team meetings Strong communication, documentation, and organizational skills Ability to work effectively with residents, families, staff, and outside agencies Knowledge of psychosocial assessments and discharge planning Ability to maintain confidentiality and handle sensitive resident information professionally Key Responsibilities Provide psychosocial support and services to residents and families Complete required social services assessments and documentation Participate in care plan and interdisciplinary team meetings Assist residents with adjustment to the facility and changes in condition Coordinate discharge planning and community resources Address resident and family concerns and assist with resolution Advocate for resident rights, dignity, choice, and quality of life Maintain accurate and timely Social Services documentation Collaborate with Nursing, MDS, Activities, Business Office, Dietary, and other departments Participate in QAPI and facility compliance activities as assigned Support the facility in meeting applicable federal and state SNF requirements What We Offer Full-Time Position Starting pay: $28.00/hour Pay negotiable based on experience and qualifications Opportunity to work with an interdisciplinary team in a 99-bed skilled nursing facility Certification and SNF experience are required.
