Applicants must have prior Social Services experience in a Skilled Nursing Facility. Candidates without Skilled Nursing Facility experience will not be considered. The Director of Social Services is responsible for planning, organizing, developing, and directing the overall operation of the Social Services Department in accordance with federal, state, and local regulations, facility policies, and established standards of care.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Plan, develop, organize, implement, evaluate, and direct the Social Services Department. Identify and address the medically related social and emotional needs of residents. Participate in resident care planning, discharge planning, and interdisciplinary team meetings. Complete and maintain timely, accurate, and informative social service documentation and progress notes. Interview residents and families to obtain social histories and identify individual needs. Assist residents and families with discharge planning, placement, transportation, community resources, and outside services. Provide residents and families with information regarding Medicare, Medicaid, financial assistance programs, and available resources as appropriate. Make referrals to appropriate community agencies and outside providers when needed. Address resident and family concerns and assist with the resolution of grievances. Maintain professional communication with residents, families, staff, physicians, healthcare providers, and outside agencies. Develop, review, and update Social Services Department policies and procedures. Maintain knowledge of current federal and California regulations affecting skilled nursing facilities and social services. Participate in facility surveys and regulatory inspections. Assist with plans of correction related to Social Services deficiencies. Participate in Quality Assurance and Performance Improvement activities. Maintain an ongoing quality assurance program for the Social Services Department. Coordinate Social Services activities with Nursing, Admissions, Activities, Business Office, Rehabilitation, Dietary, and other departments. Provide written and verbal reports and recommendations to the Administrator. Maintain required departmental records and documentation. Perform other duties related to the position as directed by the Administrator.REQUIRED QUALIFICATIONS
Bachelor's degree required. ACSW registration required. Direct Social Services experience in a Skilled Nursing Facility is required. Must have working knowledge of Skilled Nursing Facility regulations, resident rights, care planning, discharge planning, psychosocial assessments, grievances, and Social Services documentation requirements. Must have experience communicating with residents, families, interdisciplinary teams, and outside agencies in a Skilled Nursing Facility setting. Supervisory or leadership experience in Social Services is strongly desired. Strong written and verbal communication skills. Strong organizational, documentation, and time management skills. Proficient with Microsoft Word, Excel, email, and general computer applications.EXPERIENCE REQUIREMENT
This is not an entry level Social Services position. We are specifically looking for an experienced candidate who has previously worked in Social Services within a Skilled Nursing Facility. Candidates without direct Skilled Nursing Facility Social Services experience will not be considered.WAGE:

Must be at least 18 years of age. Must successfully complete required background clearance, health screening, and drug screening prior to employment. Must be able to sit, stand, walk throughout the facility, bend, reach, and perform general office and resident related duties. Must be able to occasionally lift up to 25 pounds.Pay:
