Job Description
Grand Park Convalescent Hospital is looking for a dedicated SSD provide professional social services, including, but not limited to, psychosocial assessment, patient advocacy, discharge planning, resource management, crisis intervention, community education, and supportive counseling, designed to meet the varying psychosocial needs of patients and their families as these issues relate to the current hospitalization or subsequent transition from the acute hospital setting. Qualifications 1. Bachelor's Degree in Family and Consumer Science, Social Work, Sociology, Psychology, or a related field. 2. Bilingual English/Korean or Spanish a plus. 3. Basic computer skills. Experience in Microsoft Word and Excel beneficial. 4. Effective verbal and written communication skills necessary. 5. Thorough assessment skills. 6. Demonstrated Leadership ability. 7. Must possess excellent time-management skills and be able to effectively prioritize work duties.
