Job Description

Director of Social Services — Skilled Nursing FacilityOverview The Director of Social Services leads the social work program at the facility and advocates for the emotional, social, and psychological well-being of every resident. Working as part of the interdisciplinary team, this role supports residents and families from admission through discharge — making sure each person's voice, preferences, and rights are at the center of their care.

Key Responsibilities Complete psychosocial assessments and the social services portion of the MDS 3.0. Participate in person-centered care planning with nursing, therapy, and other team members. Advocate for resident rights, dignity, and quality of life. Coordinate discharge planning to home or the next level of care. Provide emotional support around adjustment, grief, depression, and end-of-life concerns. Facilitate the Resident and Family Council and respond to grievances. Support advance care planning (advance directives, MOST form, code status). Refer residents to psychiatry, hospice, palliative care, and community resources. Supervise social services staff and maintain compliance with CMS regulations (F-Tag 850 / F-Tag 745).
Qualifications & Requirements Education:
Bachelor's Degree in Social Work (BSW), or a Bachelor's Degree in an officially recognized human services field, including Psychology, Sociology, Gerontology, Rehabilitation Counseling, or Special Education. (Master of Social Work / MSW welcomed but not required).
Licensure:
None required. Active professional licensure (LSW/LCSW) is not legally mandated for the administrative oversight of social services in a Colorado skilled nursing facility.
Experience:
Minimum of 1 year of supervised social work or case management experience in a skilled nursing facility (SNF), long-term care setting, or post-acute hospital environment working directly with individuals.
Technical Skills:
Experience utilizing electronic health records (EHR) and a strong understanding of PASRR, Medicaid, and Medicare redeterminations.
Pay:
$65,000.00 - $75,000.00 per year
