As the Social Services Director , the primary purpose of your job position is to assist in planning, organizing, implementing, evaluating, social service procerdures in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Executive Director, to assure that medically related emotional and social needs of the residents are identified and met/maintained on an individual basis to ensure that the highest degree of quality care is maintained at all times. The wage range for this position is an annual salary of $65,000 to $80,000 and is based on years of experience as a Social Worker in long term care facilities. Job Description Must have experience working in a Skilled Nursing Facility with knowledge of the MDS and PointClickCare, completing social histories, trauma screens, psychotropic medication monitoring, grievances, care conferences and discharge planning. Provide consultation to staff members, community agencies, etc., to solve the needs and problems of the resident through the development of social service programs. Review departmental complaints and grievances from employees, residents, and family members/legal representatives and make written reports to the Executive Director of action taken per facility-established procedure. Facilitate, coordinate and review residents on a quarterly, annual and as-needed basis who are ordered/administered psychotropic medications, reviewing residents' psychosocial well-being in relation to behaviors, depression, anxiety, insomnia and overall emotional well-being, establishing risks versus benefits of the psychotropic medication use. Recommend gradual dose reductions (GDR) as appropriate.
