Social Services Director North Shore Health and Rehab | Loveland, Colorado | Full-Time About the Role North Shore Health and Rehab is seeking an experienced and compassionate Social Services Director to lead our psychosocial care program. In this role, you'll oversee a team of two — a Discharge Planner and a Social Services Assistant — while ensuring residents and their families receive the support, advocacy, and resources they need throughout their stay, all while maintaining full compliance with state and federal regulations. Key Responsibilities Conduct psychosocial assessments and develop individualized care plans for residents Lead and participate in interdisciplinary care planning meetings Supervise, train, and support the Discharge Planner and Social Services Assistant, providing oversight of their daily work and professional development Oversee discharge planning processes and ensure smooth transitions for residents leaving the facility Coordinate with community resources and outside agencies to support residents/families Ensure compliance with OBRA, state, and federal regulations related to social services Investigate and respond to resident/family grievances and concerns Maintain accurate documentation in the resident's medical record (MDS Section Q involvement, care plans, progress notes) Support residents' rights, dignity, and quality of life Assist with admissions and pre-admission screening as needed Serve as liaison between residents, families, facility staff, and outside agencies Qualifications Bachelor's degree in Social Work, Psychology, or related field required (BSW preferred; MSW a plus) Colorado social work licensure as required by state regulations (please confirm current requirement) Minimum of 1-2 years' experience in a skilled nursing or long-term care setting preferred Prior supervisory experience preferred Working knowledge of OBRA regulations and MDS process Strong communication, organizational, and conflict-resolution skills Ability to work compassionately with residents, families, and interdisciplinary teams What We Offer Health, Dental, Vision Insurances PTO, 401k, 401k Matching Supportive team environment and opportunities for professional growth

$67,000.00 - $77,000.00 per year
