Overview We are seeking a dynamic and experienced Social Services Director to work and coordinate all aspects of social services and Short Term Care within our healthcare center. The ideal candidate will possess a strong working background in social work in a fast paced short term care unit, and team management, with a passion for supporting individuals with diverse needs. This leadership role requires exceptional organizational skills, relationship skills, managment skills, and a commitment to fostering an inclusive, supportive environment for our residents and families.

Key Responsibilities :

Develop and implement comprehensive social services programs that address the diverse needs of our residents Provide leadership to the Social Services team, ensuring the delivery of high-quality, person-centered care Collaborate with interdisciplinary teams to create individualized care plans that enhance the overall well-being of residents Act as a liaison between residents, families, and community resources to support a seamless transition into our facility and assist with discharge planning Advocate for residents' rights, ensuring they receive the highest standard of care and maintaining their dignity and independence Ensure staff adherence to best practices in social services delivery while fostering a culture of compassion, respect, and cultural competence. Requirements Must have a minium bachelor's degree in social work Experience in skilled nursing facility prefered
