As an HHS Case Manager II, you will help vulnerable and low-income residents access essential benefits and resources. In this role, you will determine initial and ongoing eligibility for a variety of needs-based programs, including SNAP, Medical Assistance, the Delaware Healthy Children's Program (CHIP), Subsidized Childcare, General Assistance (GA), and Temporary Assistance for Needy Families (TANF). You will work directly with customers to understand their needs, provide accurate information, answer questions, connect them with additional community resources, and help them navigate available services. This is a people-focused position where strong communication, empathy, problem-solving, and attention to detail are essential. You will provide excellent customer service, process cases accurately and timely, collaborate with coworkers and community partners, and help ensure customers receive the benefits and services for which they are eligible. JOB REQUIREMENTS
for Health/Human Service Case Manager II Applicants must have education, training and/or experience demonstrating competence in each of the following areas: Six months' experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs. Six months' experience in making recommendations as part of a client's service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits. Six months' experience in interpreting laws, rules, regulations, standards, policies, and procedures. Six months' experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data.
