Space Coast Healthcare & Rehabilitation is seeking an experienced and compassionate Director of Social Services to lead our social services department and advocate for the well-being of our residents and their families. Key Responsibilities:
Develop, implement, and maintain departmental policies in compliance with federal and state regulations Lead, mentor, and support the Social Services team Advocate for resident rights and foster a culture of dignity, compassion, and respect Participate in interdisciplinary care planning meetings Coordinate and oversee discharge planning for smooth resident transitions Monitor and improve the quality of social services to enhance resident outcomes Ensure accurate documentation and regulatory compliance Prepare and present Quality Assurance (QA) reports Serve as the facility's Grievance Officer, addressing concerns with professionalism and empathy
Qualifications:
Bachelor's Degree in Social Work (BSW) or related human services field (preferred) Minimum of 2 years of experience in a healthcare or long-term care setting Strong knowledge of federal and state long-term care regulations Excellent communication, leadership, and organizational skills
