Duties Develop and implement individualized care plans that incorporate psychosocial assessments, discharge planning, and community resource coordination to support residents' well-being. Supervise and mentor social work staff, ensuring adherence to social work practice standards, clinical ethical guidelines, and HIPAA regulations for patient confidentiality. Collaborate closely with medical teams to conduct patient assessments, diagnostic evaluations, and case management tailored to diverse cultural backgrounds and special needs such as dementia or Alzheimer's care. Facilitate individual and group counseling sessions utilizing techniques like motivational interviewing, behavioral therapy, cognitive behavioral therapy, and clinical counseling to address mental health concerns including PTSD or addiction issues. Coordinate hospice & palliative medicine services, end-of-life care planning, and crisis intervention efforts to provide compassionate support during critical moments. Maintain accurate medical documentation, medical records, and CPT coding in compliance with Medicare requirements while managing medical terminology and ICD coding for diagnoses. Lead community outreach initiatives to connect residents with external resources such as outpatient services, home & community care programs, or specialized therapies like applied behavior analysis or autism support. Requirements Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program; licensure as required by state law. Extensive experience in skilled nursing facilities or long-term care settings with a focus on geriatrics or post-acute care. Proven leadership skills with experience managing multidisciplinary teams within a healthcare environment. Strong knowledge of social work practice standards, clinical ethical guidelines, HIPAA compliance, and Medicare regulations. Hands-on experience with crisis management, discharge planning, medical documentation, and working with individuals from diverse cultural backgrounds. Certifications in CPR/BLS (Basic Life Support) required; additional certifications such as hospice & palliative care or addiction counseling are a plus.
