About Us:
Positive Care and Mentoring (PC&M) Group Home operates neighborhood-based residential programs in Columbus, GA. We provide a stabilizing, nurturing environment for youth residing outside of their family homes. Our team of Directors, HSPs, and Child Care Workers collaborate to promote safety, well-being, and permanency, helping children successfully step down to the least restrictive environment.
Position Overview:
The Human Services Professional (HSP) serves primarily in a core Case Management role. The structural scheduling for this position is divided into two potential tracks:
Full-Time Track:
Covers 2 adjacent, side-by-side programs/houses serving a combined maximum of 12 residents.
Half-Time Track:
Covers 1 program/house serving a maximum of 6 residents. You will be directly responsible for planning, arranging, coordinating, and documenting services for your assigned youth within these facilities.

Track-Based Pay:
Exact compensation scales depending entirely on your chosen track (Full-Time Track vs. Half-Time Track) and your verifiable years of human services experience.
Major Case Management Duties & Responsibilities:
Case Coordination:
Serve as the primary point of contact for DFCS case managers, court officials, educational institutions, and family members.
Service Planning:
Develop, implement, and review Individualized Service Plans (ISPs) that address safety, behavioral health, medical needs, and permanency goals.
Documentation & Compliance:
Maintain comprehensive, audit-ready physical and electronic files. Document weekly progress notes, monthly summaries, and incident reports in strict compliance with GA RBWO standards.
Admissions & Discharge:
Coordinate intake assessments for new residents and actively manage discharge planning to support successful step-downs or family reunifications.
Advocacy & Collaboration:
Lead multidisciplinary team meetings with the PC&M Director and Child Care Workers to review resident progress and adjust care plans.
Resource Referral:
Arrange and schedule external therapeutic, medical, dental, and psychological services, ensuring youth receive all designated watchful oversight supports.
Required Qualifications & Experience:
Education:
Bachelor's degree from an accredited college or university in behavioral sciences, social sciences, social work, psychology, or childhood education.
Experience:
Minimum of two (2) years of paid work experience in social services or human service delivery.
Background Clearance:
Must be able to successfully pass a comprehensive Georgia Department of Human Services / Division of Family and Children Services (DFCS) criminal background check and fingerprinting clearance. How to
