Duties:
Develop, implement, evaluate and direct the social service programs of the facility Participate in community planning related to the interests of the facility and the services and needs of the resident and family Maintain community relationships to help address resident concerns as appropriate Perform routine tasks with patient admissions, care plans and discharge planning Coordinate response to reports of missing, lost or stolen belongings Assist in obtaining resources from community and social services agencies as well as health and welfare agencies to meet the needs of the resident Develop and maintain a strong working relationship with other departments in the facility, and outside community agencies, so that social services can be provided to meet the needs of the residents. Document regarding resident social service status.
Qualifications:
Must have, as a minimum, a bachelor's degree in social work or a bachelor's degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology Social work experience in a skilled nursing/ long term care setting working directly with individuals Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public Must possess leadership ability and willingness to work harmoniously with and supervise other personnel
