Social Services Manager -AgeGuide Caregiver Resources Grant Position Description:
Provide training and education through outreach and care coordination for caregivers per AgeGuide requirements. Approximately 85% of your work hours will involve traveling to communities across Lake County to engage them in support of AgeGuide initiatives. Facilitate existing caregiver group Provide T-Care assessments Start new caregiver groups and Memory Café's Assist with data entry for the AgeGuide.Title IIIE.Caregivers Resources Assist with reporting.
