Job Description

THRYVE of Burbank is seeking a compassionate, experienced, and dedicated Social Services Director to lead our Social Services Department at our skilled nursing facility in Burbank, Illinois. In this leadership role, you will advocate for residents and their families by ensuring their psychosocial needs are met while coordinating services that promote the highest possible quality of life. The Social Services Director works closely with the interdisciplinary team to support resident well-being, regulatory compliance, and successful discharge planning. Responsibilities Direct and oversee the daily operations of the Social Services Department. Assess residents' psychosocial needs and develop individualized care plans in collaboration with the interdisciplinary team. Provide emotional support, counseling, and crisis intervention to residents and families. Coordinate admissions, transfers, and discharge planning to ensure safe and appropriate transitions of care. Assist residents and families with advance directives, community resources, financial assistance programs, and long-term care planning. Advocate for resident rights while promoting dignity, independence, and quality of life. Participate in care plan meetings, family meetings, and interdisciplinary team conferences. Maintain accurate and timely documentation in accordance with federal, state, and facility regulations. Develop and maintain relationships with hospitals, community agencies, hospice providers, and other referral sources. Supervise Social Services staff, providing coaching, training, and performance management as needed. Ensure compliance with CMS, IDPH, and applicable federal and state regulations governing social services in skilled nursing facilities. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives. Perform other duties as assigned by the Administrator. Qualifications Bachelor's degree in Social Work or a related human services field required; Master's degree preferred. Current Illinois Social Worker license (LSW) or Licensed Clinical Social Worker (LCSW) preferred, as required by applicable regulations. Minimum of 2 years of social work or case management experience in a skilled nursing facility or long-term care setting required. Previous leadership or supervisory experience preferred. Strong knowledge of Medicare, Medicaid, resident rights, discharge planning, and long-term care regulations. Excellent communication, interpersonal, and conflict resolution skills. Strong organizational skills with the ability to manage multiple priorities. Proficiency in electronic medical records and Microsoft Office applications.
