Social Services Director /
PRSD Job Summary:
We are seeking a highly skilled and compassionate Social Services Director to join our team at our Skilled Nursing Facility in Oak Park, IL. As a Social Services Director, you will be responsible for planning, directing, and evaluating the social services program, ensuring that our customers receive the highest quality of care and support. Responsibilities:
Plan, direct, and evaluate the social services program, including social services staff, direct services, advocacy, counseling, service coordination, education, in-servicing, policy, and program development, and community relations. Participate in an interdisciplinary team approach to better serve the needs of our nursing center population. Provide therapeutic intervention to help customers cope with social and psychological aspects of their illnesses, impairments, or disabilities. Maintain written documentation in the customer medical record per company policy. Develop social service components of the customer's plan of care, identifying specific problems, goals, and approaches. Coordinate discharge planning, including the development of an organized discharge plan for all customers. Ensure the safety of all center customers, adhering to legal, safety, health, fire, and sanitation codes.
Requirements:
Master's Degree in Social Work required . Possesses any certified/licensed required by State regulations. Ability to communicate effectively and appropriately both verbally and in writing in a wide variety of circumstances. Commitment to philosophy and objectives of the Skilled Nursing Facility. Knowledge of governmental regulations relating to social service delivery. Ability to work in a fast-paced environment with multiple priorities.
