Responsibilities:

Manage the coordination of services, programs, and data collection for residents, ensuring that their physical, emotional, and spiritual needs are met Provide oversight and guidance to staff, including social workers, therapists, and other support staff Develop and implement individualized care plans, working closely with interdisciplinary teams to ensure comprehensive care Facilitate skills groups, psycho-social groups, and other support groups to promote resident well-being and engagement Collaborate with families to involve them in the care planning process and provide support and education Maintain accurate and timely documentation, adhering to regulatory requirements and facility standards

Requirements:

Master's degree in human services At least 1 year of experience in a skilled nursing facility, with a focus on crisis intervention, documentation, care planning, and support groups Proven ability to work in an interdisciplinary team environment and handle confidential data with professional discretion Nursing home experience: 2 years (Required)
