Social Services Director

Job Description

Dawson Springs Health and Rehab is seeking an experienced and compassionate Social Services Director to lead our social services department and ensure the psychosocial well-being of our patients and residents. The Social Services Director oversees the development, implementation, and management of programs that address the emotional, social, and discharge planning needs of individuals receiving healthcare services. This leadership role collaborates closely with interdisciplinary teams, patients, families, community agencies, and healthcare providers to promote quality outcomes and enhance patient satisfaction. Key Responsibilities Leadership & Department Management Direct and supervise all social services functions within the facility. Recruit, train, mentor, and evaluate social services staff. Establish departmental goals and monitor outcomes and performance metrics. Ensure delivery of high-quality, patient-centered services. Patient & Resident Advocacy Assess psychosocial needs of patients, residents, and families. Develop and implement individualized care and service plans. Advocate for patients' rights, dignity, and quality of life. Assist with crisis intervention, counseling, and conflict resolution. Care Coordination & Discharge Planning Coordinate discharge planning and transitions of care. Facilitate referrals to community resources, home health agencies, hospice services, and other support programs. Collaborate with physicians, nursing staff, therapists, and other healthcare professionals to ensure continuity of care. Participate in interdisciplinary care team meetings. Regulatory Compliance Ensure compliance with CMS, Medicare, Medicaid, Joint Commission, and state regulations. Maintain accurate documentation and departmental records. Develop and update policies and procedures related to social services. Prepare for surveys, audits, and accreditation reviews. Community Relations Build and maintain partnerships with community organizations and support agencies. Educate patients, families, and staff regarding available resources and services. Represent the facility at community and healthcare networking events. Preferred Licensed Social Worker (LCSW, LMSW, CSW, or equivalent licensure as required by state regulations). Experience in long-term care, skilled nursing, rehabilitation, or hospital settings. Knowledge of Medicare, Medicaid, and managed care programs.
