Grayson Nursing and Rehabilitation is seeking a compassionate, motivated, and professional Social Services Director to join our team! Long-term care experience is highly preferred. The ideal candidate will be confident building and maintaining therapeutic relationships with residents, families, and staff while serving as an advocate for our residents and helping ensure their social, emotional, and psychosocial needs are met. Social Services Director Responsibilities Provide compassionate social services support to residents and their families. Assist with the resident admission process and pre-admission screening. Develop and coordinate effective post-discharge plans to support successful transitions. Participate in resident care plan conferences and interdisciplinary team meetings. Maintain regular contact with residents and families and schedule individual conferences as needed. Counsel and support residents, family members, and staff regarding psychosocial needs and concerns. Maintain knowledge of community resources and agencies available to assist residents and their families. Coordinate social services quality assurance activities and develop effective quality improvement programs. Process and follow up on abuse and grievance complaints, working with appropriate agencies when necessary and keeping the Administrator informed throughout the process. Initiate adult protection reports when indicated. Complete required social services forms, reports, evaluations, studies, and other documentation. Notify legally responsible parties of upcoming quarterly care plan evaluations and encourage their participation and input. Coordinate additional care plan evaluations when a resident's condition warrants more frequent review. Social Services Director Requirements Licensed Social Worker preferred. Bachelor's degree in Social Work or a related field preferred. At least one year of experience in a long-term care setting required. Strong communication, organization, and interpersonal skills. Ability to establish and maintain positive, therapeutic relationships with residents and families. Compassionate, professional, and resident-focused approach.
