Director of Social Services & Care Management Lead Transformational Social Care Across a Growing Healthcare System Are you a passionate Licensed Clinical Social Worker (LCSW) ready to shape the future of patient-centered care? We are seeking an experienced and visionary Director of Social Services to lead strategic initiatives across our Care Management department and drive innovative solutions that improve patient outcomes, strengthen quality measures, and reduce unnecessary healthcare utilization. In this highly visible leadership role, you will partner with organizational leaders, physicians, and interdisciplinary teams to create meaningful change across the continuum of care. You will oversee the social services function, inspire a high-performing team, manage departmental operations and budgeting, and champion initiatives that enhance both the patient and caregiver experience. What You'll Do Strategic Leadership & Innovation Lead the vision, strategy, and operational direction of Social Services within Care Management. Develop innovative programs and interventions that improve patient outcomes while supporting value-based care initiatives. Drive performance related to quality metrics, patient satisfaction, care transitions, and utilization management. Partner with executive leadership to identify opportunities for growth, process improvement, and service excellence. Team Leadership & Talent Development Build, mentor, and inspire a highly engaged team of social work professionals. Foster a culture of accountability, collaboration, professional development, and continuous improvement. Support recruitment, retention, workforce planning, and succession development efforts. Operational Excellence Oversee departmental performance, budgeting, resource allocation, and strategic business planning. Monitor key performance indicators and implement data-driven improvements. Ensure compliance with regulatory requirements, accreditation standards, and organizational policies. Clinical Expertise & Patient Advocacy Promote evidence-based social work practices, trauma-informed care, and patient-centered interventions. Support programs addressing behavioral health needs, crisis intervention, complex care management, and social determinants of health. Collaborate with interdisciplinary teams to improve care coordination and patient outcomes across the continuum of care. Research, Education & Growth Champion learning, innovation, and best practices across the department. Support staff education and training initiatives. Participate in strategic projects that advance organizational goals and enhance patient care delivery. Qualifications Required Education Master's Degree in Social Work (MSW) Required Experience 7+ years of progressive experience in healthcare or social services Minimum of 2 years of leadership, supervisory, or management experience Required Licensure Current Licensed Clinical Social Worker (LCSW) license, verified by the LABSWE Preferred Certifications Certified Case Manager (CCM) Accredited Case Manager (ACM) Case Management Administrator Certification (CMAC) What Makes You Successful You are a strategic and compassionate leader who brings: Strong verbal, written, and presentation skills Expertise in DSM-5 diagnostic criteria and evidence-based treatment approaches Experience with crisis intervention, suicide risk assessment, and trauma-informed care Excellent leadership, organizational, and decision-making skills Strong conflict resolution and relationship-building abilities A passion for improving patient outcomes and advancing social work excellence Why Join Us?
