This position will include the following: Evaluate patients on their needs, limitations and desires Address patient concerns and goals while maintaining constant communication with the patients and their families Build rapport with patients and their families Work with an interdisciplinary team to create care plans, address psycho-social issues and promote the quality of life Have excellent listening and communication skills A Bachelor's or Masters Degree in social work or other human services field is required.
