The Mobile Director of Social Services provides leadership, guidance, and interim social services support across a portfolio of 15 skilled nursing facilities throughout Maryland. This role ensures compliance with federal and state regulations, maintains high standards of psychosocial care, and supports facilities during vacancies, surveys, census changes, and special projects.

Key Responsibilities:

Provide interim social services coverage at assigned nursing facilities. Ensure compliance with CMS, Maryland Department of Health, and facility policies. Complete psychosocial assessments, care planning, discharge planning, and resident/family support as needed. Mentor, train, and support facility Social Services Directors and staff. Assist with state surveys, quality improvement initiatives, and corrective action plans. Collaborate with interdisciplinary teams to promote resident-centered care and positive outcomes. Travel regularly between 15 skilled nursing facilities throughout Maryland.

Qualifications:

MD LICSW

Preferred Experience in skilled nursing or long-term care social services. Strong knowledge of CMS regulations and Maryland long-term care requirements. Excellent communication, leadership, organizational, and problem-solving skills.
