This role is responsible for providing psychosocial support, discharge planning, advocacy, and care coordination while working closely with residents, families, and the interdisciplinary team. Requirements:
Qualifications & Experience Bachelor's Degree required Minimum 2-3 years of experience in long-term care and short-term rehabilitation preferred Strong conflict resolution and problem-solving skills Excellent communication and interpersonal abilities Knowledge of federal and state regulations related to long-term care Strong organizational and time management skills Compassionate, professional, and empathetic demeanor Knowledge of PointClickCare preferred
