In this leadership role, you will support the emotional, social, and mental well-being of our residents, ensuring smooth transitions and a high quality of life.

Qualifications:

Bachelor's degree in Social Work or a human services-related field (such as Psychology, Sociology, Special Education, or Rehabilitation Counseling). Minimum of one year of supervised social work experience in a healthcare setting working with elderly populations. Experience in a Skilled Nursing Facility (SNF) preferred.

Key Responsibilities:

Complete psychosocial assessments and develop individualized care plans. Maintain accurate and timely documentation in compliance with state and federal guidelines. Provide emotional support and crisis intervention for residents and families. Coordinate discharge planning and referrals to community services. Advocate for residents' rights and collaborate with the interdisciplinary care team. Educate staff and support compliance and survey readiness.
