Social Service Director Parkway Health and Rehab is an 87 bed skilled nursing facility located behind UMMC Madison in Canton, MS just off the Nissan Parkway. We are searching for a licensed social worker or social services designee for our facility.

Responsibilities include:

Plans and organizes a social service program according to current policies Assists in the coordination of resident transfers Identifies and evaluates personal, emotional and environmental concerns Formulates and updates a written plan of care for each resident Participates in the development and review of the Total Care Plan Electronic progress notes on each resident's social needs Responsible for discharge planning Shopping for residents' personal needs Manages the facility grievance process Assists with new admission referral processing in the absence of the admissions coordinator.
