Job title Director of Social Services Reports to Administrator Job purpose Responsible for overseeing the social services delivered to facility residents and families. Assess resident psycho-social needs and assists in formulation of multidisciplinary care plan. Coordinates special care unit program. Duties and responsibilities
- Conducts assigned portions of the RAI assessment (MDS) and certifies accuracy and completeness.
- Participates in the care plan process.
- Serves as Special Care Unit Program Coordinator.
- Reviews Clinical Information of prospective residents prior to bed offer.
- Assists Admissions Coordinator with PASARR process and admission/discharge duties in the absence of the admission coordinator.
- Documents in resident charts according to facility policy.
- Conducts tours in the absence of admission coordinator.
- Provides counseling services to family, residents, and staff.
- Coordinates facility psycho-social services.
- Coordinates complaints and lost items procedures.
- Coordinates planned discharges and transfers from the facility.
- Assists residents in preparation of discharge
- Ensures discharge process is completed in accordance with facility policy and federal/state regulation.
- Assists with facility investigations of abuse, neglect, and misappropriation of property.
- Participates in facility Quality Program.
- Facilitates and takes minutes for resident and family councils.
- Performs additional tasks requested by administrator or owners.
Qualifications Qualifications include:
Meets licensure requirements for Director of Social Services in North Carolina. Long Term Care experience preferred. Ability to follow directions regarding job responsibilities. Ability to perform tasks with consideration for residents in area. Ability to work and cooperate well with other employees. Commitment to customer satisfaction. Knowledge, Skills and Working Conditions Knowledge, skills and working conditions include:
- High school diploma or equivalent required Ability to read, write and understand English.
Ability to do simple calculations. Ability to follow detailed instructions. Ability and experience in counseling residents or families. Knowledge of Microsoft Office.
Supervisory / Management
skills. This job operates in a professional office environment.
Physical requirements Physical requirement include:
- Ability to sit for an extended period of time.
- Ability to lift, push, pull and carry a minimum weight of approximately 25lbs.

$55,000.00 - $60,000.00 per year
