Licensed Social Worker (CSW/LCSW/LMSW/ LSW) - Skilled Nursing Facility Edison, NJ An amazing Skilled Nursing Facility in Edison, NJ is seeking a compassionate and motivated LCSW or LMSW to join their team! Responsibilities:
Provide psychosocial assessments, counseling, and emotional support to residents and families. Coordinate discharge planning and collaborate with interdisciplinary teams to ensure smooth transitions of care. Assist residents and families with care planning, resources, insurance concerns, and community services. Maintain accurate documentation and ensure compliance with state and federal regulations. Top 3
Qualifications:
Active
CSW/LCSW/LMSW/ LSW
license in the state of New Jersey. Experience in a Skilled Nursing Facility, rehabilitation, long-term care, or healthcare setting preferred. Strong communication, organizational, and interpersonal skills with the ability to build relationships with residents, families, and staff.
Pay:
$85,000.00 - $120,000.00 per year
