Job Description
Hazlet, NJ | Skilled Nursing $85,000-$95,000 DOE Take the lead in making a meaningful difference in the lives of residents and their families. A skilled nursing facility in the Hazlet area is looking for a Director of Social Work to oversee social services and play a key role in resident support and care coordination. The Role Direct and oversee the facility's social services program Manage discharge planning and coordinate successful transitions Advocate for residents and serve as a resource for families Work alongside nursing, therapy, and other members of the interdisciplinary team Address psychosocial needs and help resolve resident concerns Maintain compliance with applicable state and federal requirements Qualifications Active NJ LSW or LCSW required Previous skilled nursing/long-term care experience required Prior supervisory or leadership experience preferred Knowledge of discharge planning and Medicare/Medicaid processes Strong interpersonal, communication, and problem-solving skills
