Competitive Base SalaryA leading Senior Living Community is seeking a Director of Social Services/Admissions to join its team. The Director will assess the psychosocial and emotional needs of residents, develop individualized plans of care, and collaborate with interdisciplinary team members to support resident preferences and overall well-being.

ResponsibilitiesMeet with new residents upon admission to review Advance Directives, resident rights, required forms, and related documentation.

Provide residents with applicable handbooks, disclosures, authorization forms, and other admission materials.

Obtain and maintain required resident and family documentation.

Assist residents with Advance Directive processes when requested.

Maintain appropriate case notes and documentation.

Provide and coordinate annual staff education regarding abuse prevention, detection, and reporting.

Develop and facilitate ongoing staff education and training related to social services, psychosocial care, and resident needs.

Participate in interdisciplinary care planning and contribute to individualized resident care plans.

Assess residents' psychosocial and emotional needs through interviews and ongoing interactions.

Advocate for resident preferences, rights, and quality of life.

Required QualificationsCollege Degree and Master of Social Work (MSW) requiredMinimum 5 years of relevant experienceMust be licensed by the New Jersey Social Work Licensing BoardExperience in senior living, skilled nursing, assisted living, memory care, or a related healthcare environment is preferred.

Strong communication, documentation, interpersonal, and organizational skills.
