Director of Social Work A leading healthcare organization is seeking an experienced Director of Social Services to join our interdisciplinary team. The ideal candidate will have experience in sub-acute and long-term care settings and a passion for enhancing resident outcomes through compassionate, person-centered care.

Key Responsibilities:

Conduct and oversee MDS-related social service assessments. Coordinate care plan conferences and facilitate discharge planning processes. Complete resident psychosocial histories, assessments, advanced directives, and end-of-life planning documentation. Review and audit social service documentation, including hospital transfers, room changes, PASRR documentation, psychological services records, and advance directives. Maintain accurate and timely documentation within the electronic medical record system (EMR). Collaborate with residents, families, and interdisciplinary team members to develop and monitor individualized care plans. Ensure compliance with resident rights, confidentiality standards, and safety regulations. Serve as a resource for residents and families regarding community services and support programs. Provide leadership and guidance to promote quality social service practices throughout the facility.

Qualifications:

Prior experience in a Skilled Nursing Facility or Sub-Acute Rehabilitation setting required. Strong organizational, clinical, and problem-solving skills. Excellent verbal and written communication abilities. Commitment to delivering exceptional customer service and resident-centered care. Knowledge of community-based resources and discharge planning processes. Experience with SigmaCare or similar electronic health record systems preferred.
