Human Services Social Work Full Job Description Position Summary:
We are seeking a compassionate, organized, and experienced Licensed Director of Social Services to lead the social services department in our skilled nursing facility. The Director of Social Services is responsible for overseeing psychosocial services, discharge planning, resident advocacy, and compliance with all federal, state, and facility regulations while promoting the highest quality of life for residents.
Responsibilities:
Direct and oversee all social services functions within the facility. Complete psychosocial assessments and develop individualized care plans. Participate in interdisciplinary care plan meetings and resident reviews. Coordinate admissions, transfers, discharges, and safe transition planning. Assist residents and families with adjustment, counseling, and community resources. Advocate for resident rights and address resident/family concerns. Ensure compliance with CMS, state regulations, and facility policies. Maintain accurate and timely documentation in the electronic medical record. Supervise social services staff and provide ongoing training and support. Coordinate family meetings and facilitate communication among residents, families, and the care team. Assist with Medicaid applications, insurance issues, advance directives, and guardianship matters as needed.
Qualifications:
Current Social Work license required (LSW preferred; state-specific requirements apply). Bachelor's degree in Social Work or related human services field required. Prior experience in a skilled nursing facility, long-term care, or healthcare setting preferred. Knowledge of CMS regulations, discharge planning, and resident rights. Strong communication, leadership, and organizational skills. Experience with PointClickCare preferred.
