In wisdom with clincal team members in facility.
Core Responsibilities:
- Conducting psychosocial assessments
- Care Planning to build indiidualized resident goals
- Discharge Coordinate to plan safe transition back home or to alternative care setting.
- Resident advocacy to protect resident rights and resolve grievances.
- Facility support to offer gudiance to families nevigating long ter care.
- Completion of Cut letters and MDS sections,
PASRR, LOC's Requirments:
- Federal and state long-term care guidelines knowledge required
- Strong Experience or license in Social Work preferred.
