Washington-Morgan Community Action is looking for a compassionate, organized, and resourceful Case Manager to join our team and support individuals and families through our self-sufficiency, housing support, and utility assistance programs . What You'll Do As a Case Manager, you will work directly with clients while ensuring program requirements and documentation are completed accurately and timely. Conduct comprehensive client assessments to identify needs, goals, and eligibility. Develop individualized service plans with measurable goals, responsibilities, and anticipated completion dates. Provide ongoing case management, advocacy, referrals, follow-up, and crisis-intervention support. Connect clients with community resources and available assistance. Educate clients and community partners about agency programs and eligibility requirements. Advocate for clients while maintaining compliance with program requirements and agency policies. Identify unmet community needs and communicate potential service gaps to agency leadership. Maintain complete, accurate, and timely electronic and paper case records. Protect client confidentiality and maintain appropriate professional boundaries. Participate in internal file reviews, monitoring visits, audits, grant reporting, and corrective-action activities. Stay current with program regulations, funding requirements, income guidelines, and agency procedures.
What We're Looking For Education:
Minimum 2-year degree in a related field .
The ideal candidate will have:
Strong communication and organizational skills. Knowledge of case-management principles and community-resource coordination. Ability to balance a direct-service caseload with detailed documentation and compliance responsibilities. Experience working with individuals or families experiencing instability, homelessness, financial hardship, or other barriers to self-sufficiency. Ability to interpret program policies, eligibility requirements, income documentation, and funding guidelines. Ability to work respectfully and effectively with individuals from diverse backgrounds. Strong problem-solving, advocacy, and follow-through skills.

Valid driver's license and auto insurance. Must successfully pass BCI/FBI background checks . Pay Based on Education We recognize the value of your education and experience.
