Social Services Director — Youngstown Area A healthcare organization in the Youngstown area is seeking a Full-time Social Services Director to join its team. The ideal candidate is passionate about resident advocacy, family support, and providing quality social services in a long-term care setting. Key Responsibilities Serve as a liaison between residents, families, and facility staff. Conduct admission interviews and social service assessments. Identify resident needs, develop Plans of Care, and monitor progress. Participate in interdisciplinary team meetings and resident care conferences. Coordinate family meetings and assist with behavioral or psychosocial concerns. Complete applicable MDS documentation. Educate residents regarding their rights and assist with advocacy. Coordinate discharge planning, home health services, equipment, and community resources. Coordinate hospice services and assist with transportation needs. Maintain community resource information and support residents in achieving their highest practicable level of functioning. Key Qualifications LSW required. Degree in Social Work preferred. One year of social services experience in long-term care preferred. Strong written, verbal, communication, interpersonal and organizational skills. Ability to work collaboratively with residents, families, and interdisciplinary team members.
