Sequoyah Pointe Living Center is seeking an experienced and compassionate Social Services Director to join our leadership team! We're looking for a motivated professional who enjoys working with residents and families and wants to make a meaningful difference and grow their career in long-term care!

What You'll Do:

Coordinate resident admissions and discharge planning. Complete required assessments, documentation, and resident interviews. Participate in interdisciplinary care plan meetings. Assist residents and families with needs, concerns, and available resources. Maintain accurate and timely documentation in Point Click Care. Advocate for residents and help promote their psychosocial well-being and quality of life.

What We're Looking For:

Long-term care experience required. Social Services experience preferred. Strong communication, organization, and problem-solving skills. Ability to work effectively with residents, families, staff, and outside providers. Knowledge of long-term care regulations and resident rights. PCC experience is a plus. Ready to make an impact?
