We're seeking a compassionate and dynamic Social Services Director to join our team. About the Role The Social Services Director plays a key role in enhancing residents' emotional and psychosocial well-being. You will work collaboratively with nursing, therapy, and administrative teams to coordinate care plans, ensure regulatory compliance, and foster a supportive, home-like environment for residents and families. Key Responsibilities Oversee social services operations within the skilled nursing facility. Conduct assessments, develop individualized care plans, and participate in interdisciplinary team meetings. Provide counseling and support to residents and families regarding transitions, discharge planning, and resources. Ensure compliance with state, federal, and CMS regulations related to social services. Lead and mentor social services staff while promoting teamwork and collaboration across departments. Build strong relationships with residents, families, and community partners to ensure continuity of care. Qualifications Bachelor's degree in Social Work or related field required. Experience in post-acute/ skilled nursing preferred. Knowledge of state and federal regulations governing skilled nursing facilities. Strong communication, leadership, and organizational skills. Passion for serving seniors and supporting quality of life.
