Immediate Opportunity Available - Social Services Manager Liberty Recruitment Partners is recruiting on behalf of a local healthcare client for an experienced Social Services Manager. We are seeking a strong, proactive leader to oversee Social Services within a highly respected senior healthcare environment. This is an excellent opportunity for an experienced social work professional who enjoys leading a team while remaining closely connected to residents, families, and interdisciplinary care partners. The Social Services Manager will provide leadership and oversight to the social work team, ensure high-quality psychosocial and emotional support for residents and families, and play an integral role in admissions, care planning, discharge planning, and transitions of care.

Key Responsibilities:

Lead, supervise, and support the Social Services team Ensure consistent, high-quality social work services for residents and families Spearhead transitions of care across multiple levels of care Partner closely with Admissions and interdisciplinary care teams Support residents and families navigating dementia care, hospice, and related resources Oversee care planning, discharge planning, documentation, and regulatory compliance Respond to resident and family questions, concerns, and changing needs Collaborate with internal departments, hospice providers, and community resources Support departmental quality and performance improvement initiatives

What We're Looking For:

Master's degree in Social Work preferred Minimum of 5 years of progressively responsible post-graduate experience in healthcare or geriatrics Minimum of 2 years of supervisory experience Long-term care experience required Working knowledge of Medicare Part A and the SNIPY process Strong knowledge of dementia care and family support Experience in a CCRC/Life Plan Community strongly preferred Experience supporting transitions of care, admissions, hospice, and interdisciplinary teams Strong leadership, communication, organization, and relationship-building skills We're looking for someone who can truly take ownership of the Social Services function —a forward-thinking leader who can support and develop a team while ensuring residents and families receive compassionate, coordinated care throughout their experience.
