Job Description

TriSource is seeking a Foster Care Case Manager to provide foster care services for children from infancy up to 21 years old. The primary goal in this role is supporting biological parents for reunification or assisting resource families to build safe, permanent homes.
Schedule:
Monday- Friday 9a-5p with adjustments as needed •2 days a week extended days are required (for example 11a-7p)
Key Responsibilities- Case Management:
Maintain a mixed caseload of 13 to 16 foster care cases and complete intakes.
Support & Compliance:
Conduct monthly in-home safety checks, manage documentation, and manage re-certifications.
System Coordination:
Log accurate data and act as a liaison with County workers.
Team Mentorship:
Help the Program Director with placements, licensing, and training workshops.
On-Call Availability:
Carry the emergency cellular phone for 24/7 coverage one week per month. Required Qualifications- Bachelor's degree in psychology, social work, counseling, education, or a human services field. 4 to 7 years of progressive experience working with children, youth, and families in a human services setting •foster care experience is a preferred Valid driver's license, reliable personal vehicle, and ability to travel within Philadelphia and surrounding counties. •mileage is reimbursed Active CPR/First Aid certification Able to lift/carry up to 20 pounds of files or comfortably hold small children during relocations.
