As Social Services Director, you will be responsible for counseling residents and coordinating any necessary psych services. You'll also be asked to participate in behavior meetings, and coordinating room changes. As required, you keep accurate records for each resident you work with, including MDS logs, and report any changes in a resident's condition to the nursing department immediately. Job Responsibilities Direct Discharge Planning Service (i.e., discharge time, special needs) and initiate a discharge follow-up call within forty-eight (48) hours after discharge. Must be able to assess, document, and report social behaviors and conflict/grievance resolution. Participate and coordinate care planning meetings, MDS completion, and psychological services. Coordinate room changes; notifying residents & families. Facilitate Resident Personal and Outside Service Needs. Direct staff training sessions such as, but not limited to: Residents' Rights, Guest Relations, and Resident Dignity. Provide discharged residents and families with placement options.
QUALIFICATIONS FOR DIRECTOR OF SOCIAL SERVICES
Previous experience in a long-term care facility preferred Bachelor's degree in Social Work or related field
