Our case managers are called Service and Support Administrators (SSAs) They to help individuals identify their goals, connect with needed services and supports, and coordinate their Individual Service Plans (ISPs). The SSA serves as the main point of contact for the individuals on their caseload. What You'll Do As an SSA, you will: Get to know the individuals you support and help them identify what is important to them. Use person-centered planning to develop, review, and update Individual Service Plans (ISPs). Coordinate services and supports with individuals, families, providers, and community agencies. Lead planning meetings and help make sure everyone is working toward the individual's goals. Help individuals and families select service providers. Monitor services, budgets, and outcomes to make sure supports are meeting the individual's needs. Help individuals participate in meaningful activities in their communities. Maintain Medicaid and waiver eligibility. Complete required case management documentation accurately and on time. Respond to concerns and complaints and follow up on incidents involving health, safety, or individual rights. Build positive, professional relationships with individuals, families, providers, coworkers, and community partners. Participate in a rotating on-call schedule to help ensure emergency support is available 24/7.
