Social Services Director Job Summary:

The Waters of Kingsport LLC is seeking a compassionate and experienced Social Services Director to join our team at the Waters of Kingsport Skilled Nursing & Rehabilitation. As a leader in providing passionate, quality care, we are dedicated to promoting a healthy environment that fosters confident communication, honor, and collaboration among all members of the resident, family, and healthcare team. Our ideal candidate will be responsible for overseeing the social services department, developing programs and services that meet the needs of our residents, and ensuring a high level of satisfaction among our residents and their families.

Responsibilities:

Oversee the social services department, including counseling, case management, and referrals Develop and implement social programs and services, such as resident activities, outings, and special events Collaborate with the healthcare team to identify and address the social, emotional, and psychological needs of our residents Provide counseling and support to residents and their families, as needed Conduct assessments and develop individualized plans to meet the unique needs of each resident Maintain accurate records and reports, including resident data and program evaluations Participate in quality improvement initiatives and contribute to the development of policies and procedures Supervise and mentor social work staff, providing guidance and coaching as needed

Requirements:

Minimum of 1 years of experience in a social services or healthcare setting Strong knowledge of social services principles and practices and ability to apply them in a skilled nursing and rehabilitation setting Excellent communication and interpersonal skills , with the ability to work effectively with residents, families, and healthcare professionals Ability to work independently and as part of a team , with a high level of flexibility and adaptability
