Full Job Description Location:
Dallas, TX Job Summary Elysian Hospice is seeking an experienced and compassionate Director of Social Services (LMSW) to lead and provide medical social work services for hospice patients and families. This role is responsible for psychosocial assessments, care coordination, patient and family counseling, and community resource coordination while working closely with the Hospice Interdisciplinary Team (IDT). The Director of Social Services will also provide leadership, mentorship, and guidance to social work staff to ensure high-quality patient care, regulatory compliance, and strong patient and family support services. This is an excellent opportunity for a social worker who is passionate about hospice, end-of-life care, and leadership while still maintaining direct patient and family involvement. Essential Responsibilities Complete comprehensive psychosocial assessments for hospice patients and families Develop and implement care plans addressing psychosocial, emotional, financial, and environmental needs Provide counseling, emotional support, and crisis intervention for hospice patients and families Coordinate community resources, financial assistance programs, and support services Collaborate with the Hospice Interdisciplinary Team (IDT) to support patient care planning and care coordination Assist patients and families with advance directives, funeral planning, discharge planning, and level-of-care transitions Provide leadership, mentorship, and guidance to social work staff and support professional development Ensure compliance with Medicare hospice regulations, documentation standards, and organizational policies Participate in IDT meetings, care conferences, and care coordination activities Participate in QAPI initiatives, education programs, and continuous quality improvement activities Promote hospice philosophy and provide compassionate, patient-centered end-of-life care Qualifications Master's degree in Social Work (MSW) from an accredited institution Current and valid LMSW license issued by the Texas State Board of Social Worker Examiners Hospice, palliative care, or healthcare social work experience preferred Supervisory or leadership experience in healthcare preferred Strong psychosocial assessment, counseling, documentation, and care coordination skills Experience working within an interdisciplinary healthcare team Proficiency with electronic health records (EHR) systems (Home Care Home Base experience preferred) Knowledge of Medicare hospice regulations and documentation requirements preferred Reliable transportation, valid driver's license, and current auto liability insurance Ability to read, write, and communicate effectively in English We are an Equal opportunity employer; We offer an excellent benefit plan to include 401K with match, CEU reimbursement, vacation, sick, holidays, medical, dental, and supplemental insurance Plans as well as a Highly competitive compensation package.
