Duties include:

Execute admitting individuals to facility working with local referral sources and community members Perform cognitive and psychosocial assessments and care planning Discharge planning to include making referrals to community resources Providing emotional support and education to residents and their family members Advocating for residents' rights

Qualifications:

Must possess a bachelor's degree in a human service field. Experience working in a nursing home setting preferred.
