Regional Director of Social Services Complete Care Careers - 2.6 Milwaukee, WI Job Details Full-time $120,000 a year 13 hours ago Qualifications Long-term care regulations Management Team leadership Long-term care facility experience Mentoring Leading team collaboration initiatives Clinical team leadership Healthcare compliance Post-acute care experience Full Job Description Complete Care Management is growing—and we're looking for a Regional Director of Social Services to lead and elevate psychosocial care across our Midwest portfolio. This role will oversee 16 skilled nursing facilities (13 in Wisconsin and 3 in the Chicago area), partnering closely with facility leadership to drive outcomes, compliance, and patient-centered care. This is an opportunity for a hands-on leader who understands the pace and expectations of post-acute care and can support, mentor, and hold teams accountable across multiple sites. Key Responsibilities Provide regional oversight of Social Services departments across all assigned facilities Support, coach, and develop facility Social Services Directors and teams Ensure compliance with federal, state, and company regulations Partner with Administrators and DONs to improve discharge planning, length of stay, and patient satisfaction Monitor and improve psychosocial outcomes and care planning processes Assist with survey readiness, audits, and plans of correction Step in to provide on-site support during vacancies or high-need situations Standardize best practices and drive consistency across the region Qualifications Bachelor's or Master's Degree in Social Work (MSW preferred) Active LCSW or equivalent licensure strongly preferred Multi-site or regional experience in long-term care / post-acute required Strong knowledge of skilled nursing regulations and survey process Proven ability to lead, mentor, and influence teams Willingness to travel regularly throughout Wisconsin and Illinois Why Join Complete Care Opportunity to lead across a large and growing regional footprint Strong operational and clinical support structure High-impact role with direct influence on patient outcomes and facility performance Competitive salary based on experience If you're a strategic, hands-on Social Services leader ready to make an impact across multiple facilities, we want to connect with you. #LI-LA1 #CC2024
